Your First Visit
at Smile Exchange

Complete New Patient Forms
If you are able to fill out your new patient forms prior to your visit, it will save time as you can send them directly to us or bring them to your appointment. The forms are available here. If unable, that is okay but we ask that you arrive 15 minutes prior to your new patient appointment so we can allow for registration.
X-Rays
New patients receive a comprehensive new patient examination and most commonly take a full mouth series of X-rays at the initial visit, or those X-rays that the doctor feels necessary for proper diagnosis. X-rays are important for us to be able to see a complete picture of your teeth, gums, and jaw to ensure proper diagnosis, if any concerns exist.
Comprehensive New Patient Exam
This appointment is focused on getting to know you and completing the examination to help you find a happy, healthy smile. After your dental and medical history is reviewed and discussed, your dentist at Smile Exchange will visually examine all of your teeth and review the X-rays, complete an oral cancer screening, and our hygienist will examine your gums and bone levels. We will take the time to discuss your treatment goals and provide you options for any treatment you may have.
